Application Extracurricular Classes

I understand and acknowledge that participation in STEAM extracurricular activities, including but not limited to science experiments, technology use, engineering projects, art projects, and mathematics exercises, may involve inherent risks. These risks may include, but are not limited to, physical injury (such as cuts, burns, allergic reactions), illness, or damage to personal property.
I voluntarily assume all risks, both known and unknown, related to my child’s participation in these activities and understand that, despite reasonable efforts made by the School and its staff to ensure safety, accidents or incidents may occur.

In consideration of my child’s participation in the STEAM extracurricular activities, I, on behalf of myself, my child, and our respective heirs, successors, and assigns, hereby release, waive, and discharge Schola International, its owners, directors, employees, instructors, volunteers, and agents from any and all claims, demands, liabilities, or causes of action, including those arising from the negligence of the Releasees, that may arise out of or relate to any injury, illness, or loss that occurs during or as a result of my child’s participation in these activities.
I further agree that the Releasees will not be held responsible for any direct or indirect injury, loss, or damage incurred as a result of participation in the STEAM classes.

I agree to indemnify and hold harmless the Releasees from any and all claims, liabilities, damages, or costs (including legal fees) arising from my child’s participation in the STEAM extracurricular classes, whether caused by the negligence of the Releasees or otherwise. This includes, but is not limited to, claims brought by third parties as a result of my child’s participation in these activities.

In the event of an emergency, I hereby authorize the School to seek medical attention for my child if I cannot be reached. I agree to be fully responsible for any medical costs incurred as a result of any injury or illness sustained by my child during participation in STEAM activities. I also acknowledge that the School does not provide medical insurance for participants in extracurricular activities.

I understand that the School will take reasonable precautions to provide a safe environment, and I agree that my child must follow all safety instructions and behavioral guidelines set by the instructors. Should my child fail to follow these guidelines, I understand that the School reserves the right to remove my child from the activity for their safety and the safety of others.

I acknowledge that during STEAM activities, my child may use materials, tools, and equipment (such as scissors, art supplies, technological devices, or small construction tools) that may pose a risk if not used properly. I understand that the instructors will provide guidance on the correct and safe use of all equipment, and I agree to hold the Releasees harmless for any misuse of such materials by my child that may result in injury.

I grant permission for Schola International to use photographs, videos, or other media that include my child, taken during participation in STEAM extracurricular activities, for promotional, educational, or marketing purposes without any compensation.

I have carefully read and fully understand this Liability Release, including the assumption of risks and waiver of liability. I acknowledge that I am signing this document freely and voluntarily, and that I fully intend, by my signature, to release the School and its staff from any liability for injuries, damages, or losses incurred by my child during participation in the STEAM extracurricular activities.
